There are many levels of care and assessments are need to insure the best care possible. These assessments are conducted every
90 days, this isn't just good business practice, it's the law. Below is an assessment tool that can help get you started to
determine you or your family members needs.
Examples of the levels of Care.
Level One Care
1. Bathing Assistance once weekly.
2. Personal laundry assistance weekly
3. Medication monitoring & reminder program
4. Occasional dressing assistance & clothes setup
5. Assistance scheduling medical appointments
6. Help with modified diets
7. Occasional assistance with grooming & hygiene
8. Daily bed making
Level Two Care
All services included in Level One, plus
1. Bathing assistance more than once each week but not daily
2. Daily assistance with hygiene, dressing & grooming
3. Occasional wheelchair assistance
4. Nutritional intake & eating habits closely monitored
5. Assistance with incontinence
6. Social or emotional support
Level Three Care
All services included in Level One and Two, plus:
1. Daily or more frequent assistance with bathing
2. Daily or more frequent assistance with dressing, hygiene & grooming
3. Wheelchair assistance or escort to all meals and activities
4. Regular assistance with mobility& toileting
5. Nighttime assistance (toileting, reassurance, etc.)
6. Intensive incontinence management
